For mushroom ceremonies

When making a reservation for one of our events, I declare that:

I am participating in this ceremony out of free will;

I do not take any medication that is incompatible with magic truffles, specifically I do not use any anti-depressive and anti-psychotic medication;

I am in a physical, emotional and mental condition that is compatible with using magic truffles, specifically I do not have any suicidal tendencies;

I am least eighteen (18) years old;

I am not under the influence of narcotics or alcohol;

I understand that this ceremony is not offered as an alternative for medical treatment;

I will follow all guidelines given regarding this ceremony, before, during and after the ceremony;

I understand that this ceremony is organized by ‘NatuurKrachtCoach’;

I have been informed thoroughly about the ceremony and the effects of using magic truffles;

I acknowledge that participating in this magic truffle ceremony is my own responsibility.

I acknowledge that everything that happens during a group magic truffle ceremony is of a private nature. I understand that I cannot share or publish anything that happened during the ceremony or retreat without explicit permission of NatuurKrachtCoach.

In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my heirs, next of kin and assigns as follows:

A) I waive, release and discharge from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me ‘NatuurKrachtCoach’ and/or their directors, employees, volunteers and representatives, the activity or event holders, activity or event volunteers, owners of the location of the venue;

B) Indemnify, hold harmless and promise not to sue the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise.

I acknowledge that ‘NatuurKrachtCoach’ and their directors, volunteers, representatives, owners of the venue of the location and agents are NOT responsible for the errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of ‘NatuurKrachtCoach’.

I have fully understood everything that has been written in this document an all documentation that I have received before.
